» Submit Copy of diploma / STTB legalized original 2 pieces.
» Submit a certified Copy of the original NEM 2 sheets. | | ◨ | For graduate D3/D2/D1/S1 :
» Submit Copy of diploma legalized original 2 pieces.
» Submit Copy of Transcript of legalized original 2 pieces. | | ◨ | For Transfer Students:
» Submit Copy of diploma / STTB SLTA legalized original 2 pieces.
» Submit Transcript of Origin legalized Campus original 2 pieces.
» Moving Certificate (From Campus of Origin). | | ◨ | Submit recent photographs of size 2 x 3, 3 x 4, 4 x 6 (5 pieces each) - be followed | | ◨ | Bring stamp IDR 6,000 (1 piece). |
